Congress didn't renew temporary enhanced Affordable Care Act federal subsidies that expired at the end of 2025, a move that substantially raised ACA health insurance premiums.

Findings

Additional insights we found via KFF

  1. Before the expiration of the ACA subsidies, about 24 million people had health insurance through the ACA marketplace.

  2. KFF analysis had estimated that if the enhanced premium tax credits were not renewed, ACA premium payments could rise 114% on average — more than doubling costs for many enrollees.

  3. Hospitals announced in July 2026 that the number of uninsured patients increased almost 21% from 2025, citing the lapse of the ACA federal subsidies as people decided they couldn't afford the higher premiums.

  4. People also switched to high-deductible plans to lower their monthly premiums, but that led to higher out-of-pocket costs they sometimes couldn't pay, leaving hospitals with an almost 37% increase in bad debt they won't be able to collect.
